Passporting

I.

A. Requirements for first time applicants

Duly accomplished Agency Application Form.
Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO( If NSO BC is not readable)
Proof of identity / Supporting documents indicating full name, date and place of birth and citizenship

Other Supporting documents:

NSO Marriage Contract (for married woman),  Voter’s Registration Record or Voter’s ID, PRC / IBP ID,  Baptismal Certificate, Seaman’s Book, Driver’s License, Digitized SSS-ID, NBI (old copy), School Form 137 or Transcript of Records, School ID, Other documents that show full name and birth details of applicant and citizenship.

B. For Minors

Personal appearance of either parent (if minor is of legitimate status).
Personal appearance of mother (if minor is of illegitimate status).
Duly accomplished Agency Application Form.
Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO( If NSO BC is not readable)
NSO Marriage Contract (MC)of the parents and photocopy
Passport of parent who will appear in DFA and Photocopy

II.

A. Requirements for renewal of passport

Duly accomplished Agency Application Form.
Original Passport
Proof of Identity

B. For Minors

Personal appearance of either parent (if minor is of legitimate status).
Personal appearance of mother (if minor is of illegitimate status).
Duly accomplished Agency Application Form.
Photocopy of Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO)
Photocopy of NSO Marriage Contract (MC) of the parent
Passport of parent who will appear in DFA and Photocopy

NOTE for First Time & Renewal Applicant: Personal Appearance is required (from Infant to Senior Citizen)
